L1.1 The 7.08SL Boom Conveyor Drive fluid coupling fusible plug reached its temperature limit, resulting in the release of the entire capacity of it's fluid system (22.5Litres). EPA to monitor future compliance with this condition 1
L1 An underground fire water distribution line located at K7 wharf at the Kooragang Coal Terminal (KCT) was found to have broken resulting in process water making its way to the surface and discharging to the harbour. Appropriate Action taken by licensee 1
L1.1 During operator changeover at 7.08 Shiploader, a small leak from the end of the boom launder was observed. An inspection identified a restriction in the launder causing water containing fines to overtop the launder when luffed above zero degrees EPA to monitor future compliance with this condition 1
L1.1 On operator changeover at 7.09 Shiploader, the off-coming operator identified a dark coloured plume in the harbour below the Shiploader. Investigations identified a seal had failed at the junction of the Shiploader boom underpan and launder Appropriate Action taken by licensee 1
O7.1 After completing a task onsite, a franna has departed site and dropped/tracked mud from its tyres onto Curlew Street. Appropriate Action taken by licensee 1
O7.1 A semi-trailer was reversing our of site from Gate 32 with the aide of two spotters. The truck had unknowingly left the roadway onto a grassed verge, causing mud to be tracked down an access ramp and onto Cormorant Road. Appropriate Action taken by licensee 1
